Manager of Quality Care Initiatives
Rob Schwartz, LCSW
Originally from Brooklyn, New York, Rob received his MSW from the University at Buffalo School of Social Work and has over 30 years of administrative, managerial, and clinical experience working in various non-profit human service and healthcare organizations. Prior to joining the SMG team, Rob was Director of Psychosocial Support Services for Hospice Buffalo, where he worked in collaboration with the interdisciplinary medical team, providing clinical and administrative oversight for homecare and nursing home social workers, the Hospice bereavement program, and Life Transitions Center. He also was the Vice-President of Community & Workplace Services at Child & Family Services for over 25 years, where he was responsible for management of an EAP, administrative oversight of several other community based programs, management and training for the agency’s Quality Improvement Program, and administrative leadership and support for the agency’s national re-accreditation process. Along with his administrative and managerial background, Rob has had clinical experience providing individual, marital, family and group counseling in an alcoholism rehab program and family service agencies. As Manager of Quality Care Initiatives, Rob is responsible for administrative oversight of quality measures established by Medicare and other commercial payers, preparation for SMG’s re-accreditation process as a Patient Centered Medical Home and other quality initiatives (e.g. Chronic Care Management, CPC+). Rob previously was SMG’s Manager of Behavioral Health and continues to provide consultation and support for patients and staff, on an as needed basis.
